choose your individual test or panel, create patient portal, complete consent form and any Questionnares, pay for lab. Provider would review and order lab. You would receive link via email with Lab requisition and additional instructions. Go to lab location for blood draw, receive result in days. Share with Provider of choice, and /or primary care provider.(Lab test services available in all states except NY due to State laws. )

-3% to all patients over 65 years of age, full-time/part-time teachers, active patient Care Technicians (PCTs), active Medical Assistants (MAs), active Certified Nursing Assistants/Geriatric Nursing Assistants (CNA/GNA), Ministers, Active-duty military and Veterans.

-5% to active Janitors/Housekeepers, active Trash/Garbage collectors, Farm workers/roles like Farm Hand, Laborers, Planters/Harvesters/Pickers, road construction worker (unskilled) such as General Laborer.

This discount applies only to weight loss medication prescription. This discount may not be combined with any other discount or promotions.
It is your responsibility to provide evidence of your Job, Role, and or, Tasks by way of job offer, ID, paystub, employer, work hours, etc.
